										<content:encoded><![CDATA[<p>Una crisis en evolución con éxitos y nuevas dificultades es lo que se describe en cuatro informes publicados por el Health Department, en los que se describe el consumo de sustancias y sobredosis en la ciudad de <a href="http://www.phila.gov" target="_blank" rel="noopener">Philadelphia</a> entre 2022 y 2023.</p>
<p>Palak Raval-Nelson, comisionado de Salud de Philadelphia, destacó el arduo trabajo realizado para que en los informes de 2023 se muestren un progreso real. “No entramos en la crisis de sobredosis de la noche a la mañana, y no vamos a salir de ella de la noche a la mañana”.</p>
<p><strong>En 2023, Philadelphia experimentó una disminución del 7% en las muertes por sobredosis y los datos preliminares muestran una disminución adicional en 2024. Sin embargo, la disminución del año pasado no se distribuye equitativamente.</strong></p>
<p>Si bien hubo una disminución del 15% en las muertes por sobredosis entre las personas blancas no hispanas, solo hubo una disminución del 5% entre las personas afroamericanas no hispanas y hubo un aumento del 2% entre las personas hispanas.</p>
<p>En respuesta a esto y a otros cambios en la forma de la crisis de sobredosis, la ciudad continúa evolucionando. Desde programas de alcance puerta a puerta en los vecindarios afectados hasta la distribución de tiras reactivas y naloxona, y programas de servicios ambientales que recogen miles de bolsas de basura en los vecindarios afectados.</p>
<h2><strong>Las drogas están cambiando en esta crisis de sobredosis</strong></h2>
<p>Además de los cambios demográficos de la crisis de sobredosis, las drogas involucradas están cambiando. Esta crisis se caracteriza no solo por el fentanilo, sino también por los estimulantes.</p>
<p>Según un comunicado, en 2023, el 80% de todas las muertes relacionadas con drogas involucraron fentanilo, el 63% involucraron cocaína y el 15% involucraron un estimulante sin opioides.</p>

<p>Por otro lado, la introducción del sedante veterinario xilazina, asociado con heridas graves, estuvo implicada en el 38% de las muertes por sobredosis en 2023, y siempre estuvo relacionada con el fentanilo.</p>
<p>En respuesta, el Health Department ha implementado un programa de atención de heridas y continúa colaborando con hospitales y centros de tratamiento de la zona para tratar tanto las heridas asociadas con el consumo de sustancias como los trastornos por consumo de sustancias.</p>
<p><strong>Como parte del trabajo continuo de reducción de daños de la ciudad, el departamento distribuyó más de 100.000 dosis de naloxona en 2023, casi el doble de la distribución de 2022. Los residentes pueden obtenerla gratuitamente del Health Department, así como con receta o sin receta en farmacias locales.</strong></p>
<p>Además de la naloxona, el departamento ha distribuido más de 180.000 tiras reactivas de fentanilo y casi 50.000 tiras reactivas de xilazina. La ciudad ofrece capacitación gratuita regularmente sobre cómo usarlas, como sobre cómo revertir una sobredosis con naloxona.</p>
<p>El Health Department trabaja en estrecha colaboración con los proveedores médicos, quienes están en la primera línea de la crisis de sobredosis. Más de 10.000 pacientes con trastorno por consumo de opioides son atendidos cada año en los servicios de urgencias de Philadelphia. <p><strong>Una declaración de Emergencia de Salud por Calor activa los programas de emergencia de calor de la ciudad, que incluyen la línea de calor de la Corporación para el Envejecimiento de Philadelphia, centros de enfriamiento, visitas domiciliarias por parte de equipos de campo especiales, mayor alcance diurno para personas sin hogar y el recordatorio al público para que controle de manera segura a sus amigos, parientes y vecinos mayores desde la distancia.</strong></p>

<h2><strong>Centros y lugares de enfriamiento por Emergencia de Salud por Calor</strong></h2>

<h2><strong>Asistencia social y refugio </strong></h2>
<p>La Oficina de Servicios para Personas sin Hogar también declaró un Código Rojo a partir de las 7: 00 del sábado 13 de julio y tomará medidas proactivas para proteger a los habitantes que están sin hogar.</p>

<p>El Health Department recomendó que, para evitar enfermedades relacionadas con el calor, los habitantes de todas las edades deben:</p>
<ul>
<li>Utilizar aire acondicionado. Si es necesario, acuda a un lugar durante las horas más calurosas del día.</li>
<li>Si usa un ventilador, asegurarse de abrir las ventanas para liberar el aire caliente atrapado.</li>
<li>Beber abundante líquido, especialmente agua, para evitar la deshidratación. Evitar la cafeína y el alcohol.</li>
<li>Nunca dejar a personas mayores, niños o mascotas solos en el automóvil.</li>
<li>Las personas que toman medicación de forma habitual deben consultar con su médico, ya que algunos medicamentos provocan reacciones adversas en climas cálidos.</li>
<li>Usar ropa ligera y de colores claros.</li>
<li>Evitar, en la medida de lo posible, trabajar o jugar bajo el sol o en otras áreas calientes, especialmente durante las horas pico de sol, de 11:00 am a 4: 00 pm</li>
<li>Mantener una dieta normal.</li>
<li>Dúchese o báñese con agua que esté cerca de la temperatura de la piel.</li>
<li>Cubrir toda la piel expuesta con un protector solar FPS, 15 o superior. Usar un sombrero de ala ancha para proteger tu rostro y cabeza, y aplica protector solar debajo de la mascarilla.</li>

<p><strong>La alcaldesa Cherelle Parker destacó que Bettigole ha servido bien a la ciudad y a los ciudadanos como comisionada de salud, por lo que le agradeció toda su entrega y servicio público a Philadelphia.</strong></p>
<p>“El <a href="https://www.phila.gov/es/departments/department-of-public-health/" target="_blank" rel="noopener">Health Department</a> brinda servicios vitales para nuestros residentes, desde atención primaria hasta vacunas y alertar a Philadelphia cuando se están propagando enfermedades transmisibles, entre muchos servicios, y aceptamos su misión. <p>Franklin, quien estará al frente de departamento mientras se lleva a cabo una búsqueda a nivel nacional de un nuevo director médico del Health Department, se ha desempeñado como comisionado adjunto durante los últimos tres años.</p>

<p>Recibió su formación doctoral en la Escuela de Salud Pública Bloomberg de la Universidad Johns Hopkins, obtuvo una maestría en salud pública y completó una formación posdoctoral en políticas de salud en la Facultad de Medicina de Morehouse.</p>
<p><strong>Según un comunicado de prensa, Franklin también tiene un doctorado en Jurisprudencia de la Facultad de Derecho Kline de la Universidad de Drexel, y es ex miembro de la Junta Nacional de Consejeros Científicos del Centro Nacional para el Control y la Prevención de Lesiones de los Centros para el Control y la Prevención de Enfermedades. </strong></p>
<p>Anteriormente, Franklin se desempeñó como director de la División de Epidemiología, Análisis y Evaluación en el Departamento de Salud del Condado de Multnomah.</p>
<h2><strong>Bettigole, tres años como comisionada de Salud</strong></h2>
<p>Bettigole ocupó el puesto de comisionada de Salud interino en mayo de 2021 y nombrada directora del departamento en noviembre de ese mismo año.</p>
<p>Durante su mandato, supervisó el desarrollo de un plan estratégico de cinco años centrado, entre otras cosas, en la equidad en el trabajo del departamento, mejorar el acceso a la atención primaria y prepararse para futuras emergencias de salud pública.</p>

<p>Bettigole también dirigió los esfuerzos del departamento para responder a múltiples eventos de personas desplazadas en masa, incluyendo evacuados y migrantes afganos y ucranianos transportados en autobús desde la frontera sur, colaborando con socios locales, estatales y federales para crear nuevos modelos de atención para las poblaciones migrantes.</p>
<p>El departamento además renovó su acreditación a través de la Junta de Acreditación de Salud Pública y el Proyecto Public Health Ready bajo su dirección.</p>

										<content:encoded><![CDATA[<p>El personal de control de vectores del <a href="https://www.phila.gov/departments/department-of-public-health/" target="_blank" rel="noopener">Health Department</a> aplicó un tratamiento larvicida para el control de mosquitos en el área entre las avenidas West Nedro y West Godfrey; y entre la calle 5 y las vías del tren.</p>
<p><strong>Este spray es parte de un programa financiado por el estado para reducir el número de mosquitos en áreas que han sido positivos para el virus del Nilo Occidental en los últimos años. </strong></p>
<p>Cada temporada, el Health Department trabaja para reducir el número de larvas de mosquitos mediante tratamiento aplicado en las entradas de alcantarillado y, además, alienta a los residentes a verter agua estancada.</p>
<p>Según un comunicado de prensa, ciertas especies de mosquitos son portadores del virus del Nilo Occidental, que puede causar encefalitis, una infección que puede provocar la inflamación del cerebro.</p>

										<content:encoded><![CDATA[<p>Mayor Jim Kenney has appointed Dr. Cheryl Bettigole as Health Commissioner. Bettigole has been serving as the interim commissioner of the city’s Department of Public Health since May, when former department head Dr. Thomas Farley resigned at the request of the mayor.</p>
<p>She is a board-certified family physician with a doctor of medicine degree from Thomas Jefferson University. She also holds a master’s of public health from Johns Hopkins University.</p>
<p>According to WHYY, before working at the Health Department, Bettigole saw patients at a Federally Qualified Health Center serving immigrants in southern New Jersey, and as a doctor and clinical director at the city’s health centers she treated patients for over a decade.</p>
<p>“Throughout her entire career, Dr. Bettigole has demonstrated a deep commitment to prioritizing equity, access, and prevention in public health,” Kenney said in a statement.</p>
<p>The Health Commissioner keeps racial equity as a priority for her new position. She plans to hire the city’s first chief racial equity officer and create a health equity plan for the Health Department.</p>

<p>Bettigole aims to increase access to primary care because it was clear how much easier it was during the pandemic for people to access health care when they had doctors they trusted.</p>
<p>In addition, she hopes to rewrite several of the city’s emergency planning documents, those for mass death events, another pandemic, or a toxin release, for example using an equity perspective.</p>
<p>“Unless we have a very, very intentional focus on equity before all else, the general forces of structural racism are such that the whole system pushes away from equity,” she said.</p>
<p>A lesson learned</p>
<p>Bettigole considers the real impact of those forces as a lesson learned from the pandemic. “It’s not that we didn’t understand how powerful those forces were, because we work in public health and you come up against that all the time. But the scope of that, and how much ingenuity and planning and sheer resources that takes, is probably the greatest thing we learned from the pandemic.”</p>
<p>Until her appointment as interim commissioner, she was director of the Division of Chronic Disease and Injury Prevention for the Health Department. In that role, which she had held since 2015, Bettigole worked on the regulation of tobacco products and the establishment of a new injury prevention program focused on the prevention of gun violence, among other efforts.</p>

										<content:encoded><![CDATA[<p><strong>The City of Philadelphia on Thursday released vaccination rates for its employees, divided by department. The data were compiled through a combination of records submitted by employees along with other vaccine records maintained by the Health Department.</strong></p>
<p>The rates only include workers who received their doses in Philadelphia.</p>
<p>According to WHYY, the most vaccinated city department is the Law Department, where 91% to 100% of workers are vaccinated. <strong>Also among the departments with the highest vaccination rates were the Free Library of Philadelphia, the Mayor’s Office, Innovation and Technology, and the Department of Public Health, all ranging from 81 to 90% vaccinated as of Thursday.</strong></p>
<p><strong>The least vaccinated department was the Streets Department, where just 41% to 50% of workers are vaccinated.</strong></p>
<p>Between 51 and 60% of employees are vaccinated in both the Philadelphia police and fire departments. That range represents a jump from just over a week ago when the mayor’s office reported that only 13% of police officers were vaccinated.</p>

<p>Health Department spokesman James Garrow said the reported percentages were for the number of people in those departments who submitted proof of vaccination to the city’s human resources system. This percentage is based on actual records maintained by the city and does not rely on workers reporting their vaccination rates.</p>
<p><strong>The current Philadelphia mandate requires any city employee who is not vaccinated to double mask at all times.</strong></p>
<h4><strong>Great progress</strong></h4>
<p>“Our goal is [to] make sure that everyone in the city, employees and otherwise, gets vaccinated,” said Acting Health Commissioner Dr. Cheryl Bettigole in a statement. “While we’re making great progress on that, there’s still plenty of work to do.”</p>
<p><strong>Bettigole added that to help encourage city employees to get vaccinated, the Health Department will continue to provide educational resources regarding vaccine safety and effectiveness.</strong> As for all Philadelphians, to place vaccine clinics in the areas of the city with the lowest vaccination rates. Non-uniformed employees also get four hours of time off if they upload their vaccination status to the city’s system.</p>

Tom Wolf said during a news conference at a vaccination site in Bradford County that a full reopening of businesses in the state could happen two weeks after 65-70% of the state’s population is vaccinated for COVID-19, allowing that extra time for “herd immunity.”</strong></p>
<p>When asked if he had a metric to gauge when a 100% reopening could occur, the Governor explained that he has no single metric or statistic in mind. He also noted that the federal government has no fixed target, according to the Morning Call.</p>
<p><strong>“I think the general idea is that somewhere around 65-70% of the population getting both doses, two weeks after that you have herd immunity,” Wolf said.</strong></p>
<p>However, he was optimistic about a shorter time for full reopening if the residents follow the security measures to prevent contagion. “But to the extent we have people willing to wear masks, practicing social distancing and doing the things that can offset the disease, we can actually go faster on that.”</p>
<p>Wolf´s projections occur when over 26.5% of the state population already has received two doses.</p>

<p><strong>The measures that have been issued by his administration establish 25 % maximum occupancy for indoor gatherings and 50% for outdoor events; regardless of venue size and only if attendees and workers can stay 6 feet apart.</strong></p>
<p>All in-person businesses are allowed to operate at 75% occupancy, with exceptions, but they are “strongly encouraged” to conduct operations remotely, if possible.</p>
<h4><strong>Vaccines to reach full reopening</strong></h4>
<p>In order to be confident about a sooner full reopening, Wolf urged residents to get vaccinated.<strong> He said each person who gets the shots makes things safer for everyone. “All of us get a little bit closer to that normal that we want to get back to,” he said.</strong></p>
<p>Meanwhile, a significant percentage of residents either may not or will not get the vaccine. A Muhlenberg College Public Health Program poll found that as of late March, about 3 in 10 Pennsylvania adults who were not immunized indicated they did not plan to get vaccinated. Recent data from the Health Department showed only 53% of staff in skilled nursing facilities sought the vaccine even after months of availability.</p>

										<content:encoded><![CDATA[<p><strong>Pennsylvania Health Department Secretary Dr. Rachel Levine held a briefing Monday at 11:30 a.m., where she discussed the performance of COVID-19 in the commonwealth and provided an update on the Early Warning Monitoring Dashboard.</strong></p>
<p>According to an ABC33 report, the Health Department announced a two-day increase of 8,663 positive cases of COVID-19 and 203 new deaths since Saturday’s report, where 7,174 new cases and 26 new deaths were accounted.</p>
<p><strong>There were 3,779 additional positive cases as of Monday, and 4,884 cases reported Sunday. That brings the statewide total to 613,804 cases and 15,086 deaths attributed to COVID-19.</strong></p>

<p>The Health Department states that there are 5,905 individuals hospitalized with COVID-19, near double the peak in the spring. Of that number, 1,145 patients are in the intensive care unit with COVID-19. Most of the patients hospitalized are ages 65 or older, and most of the deaths have occurred in patients 65 or older.</p>
<h4><strong>Hospitalized patients</strong></h4>
<p><strong>The trend in the 14-day moving average of number of hospitalized patients per day has increased by nearly 5,600 since the end of September.</strong></p>
<p>Statewide percent positivity for the week of December 18 to December 24 stood at 15.1%.</p>
<p>Health Department Secretary Dr. Rachel Levine encourages Pennsylvanians to stay vigilant and continue taking precautions to keep themselves safe, including social distancing and wearing masks.</p>

<p><strong>There are 48,136 individuals who have a positive viral antigen test and are considered probable cases and 639 individuals who have a positive serology test and either COVID-19 symptoms or a high-risk exposure.</strong></p>
<p>There are 3,238,091 individuals who have tested negative to date.</p>
<p>As stated by the Health Department in nursing and personal care homes, there are 51,420 resident cases of COVID-19, and 9,219 cases among employees, for a total of 60,639 at 1,457 distinct facilities in all 67 counties. Out of our total deaths, 8,470 have occurred in residents from nursing or personal care facilities.</p>

<h4><strong>Who does not need a test?</strong></h4>
<ul>
<li>People who do not have symptoms of COVID-19 (no fever, or cough, or shortness of breath).</li>
<li>People with only fever and mild cough.</li>
<li>People who are otherwise healthy and are not feeling short of breath or experiencing worsening cough and fever. People who are sick but do not need testing should stay at home away from other people until their symptoms are better.</li>
</ul>
<h4><strong>Where can someone get tested?</strong></h4>
<p>If you think you should be tested for COVID-19:</p>
<ul>
<li>Start with your regular medical provider or the hospital system where you last received medical care. Call or contact them over the internet so that a medical professional can evaluate whether testing is appropriate for you.</li>

<h4><strong>What should I do while I am waiting for my test results?</strong></h4>

<li>Depending on what laboratory performs your test, it may take 2-6 days to learn your results. During that time, get rest, stay hydrated and stay at home, away from all people while you have symptoms.</li>
<li>Monitor for symptoms – if you feel short of breath or have a worsening cough or persistent fever (over 101 for more than 3 days or over 100.4 for more than 5 days), call your doctor or go to a hospital emergency department.</li>
